If you are seeking an alcohol & drug evaluation, contact your counselor to schedule an appointment.
The counselor will provide you with a password that opens sections 1 and 2. Open section 1, enter the password, complete the questions and click submit. Now follow the same procedure for section 2. Then your counselor will open section 3 and conduct a clinical interview, review your answers, and explain the results. You will receive an email with the assessment results, and any recommendations if needed. |
